Waivers, Medical Forms & Legal Agreements
Complete your paperwork before arriving at the shop or pool. All student divers, rental customers, and trip participants must sign the applicable forms below.
Student & Course Forms
Required for ALL diving students. If you answer YES to any medical condition, physician clearance is required before pool sessions.
Statement of understanding of safe diving practices. Signed during your first class session.
Acknowledgment of risks and independent contractor status. Provided and signed in-store at course check-in.
Parental consent and youth-specific medical questionnaire required for Junior Open Water (ages 10–14) and Bubblemaker / Seal Team (ages 8+). Provided at check-in.
Equipment & Rental Waivers
Required when dropping off regulators, BCDs, or cylinders for service, overhaul, or hydrostatic testing with Airtech Scuba Services.
Must be signed before any rental gear release. Covers BCD, regulator, tank, wetsuit, and dive computer rentals. View rental rates here.
Travel & Trip Agreements
Required for all Ascuba Diving group trips and liveaboard bookings. Covers deposit terms, cancellation policy (60-day notice), and travel insurance recommendations.
We strongly recommend DAN dive accident and trip insurance for all travel. Coverage starts at ~$40/year.
Technical Diving Forms
Additional liability acknowledgment required for all Tec 40, Tec 45, Tec 50, and Poseidon Se7en+ CCR course enrollments. Provided at course orientation.
Important Notes
- Forms marked with external links open on PADI.com or DAN.org and can be completed digitally.
- All other forms are provided in-store at check-in and require a wet signature.
- Students under 18 must have a parent or legal guardian co-sign all waivers.
- Medical clearance letters must be from a physician — walk-in clinic forms are accepted.
